You will receive visit notes via fax, and the Bridge tool will show real-time updates on referral progress and patient enrollment status. 

Who provides the care?

All care is delivered virtually by Sylvan Health’s licensed Registered Dietitians trained in evidence-based medical nutrition therapy.

What types of patients are eligible?

Patients managing digestive, metabolic, or lifestyle-related conditions – such as IBS, diabetes, obesity, cardiovascular concerns, or other inflammatory conditions – are typically eligible for Sylvan Health’s nutrition programs. 

What kind of support is provided for patients?

Patients receive individualized nutrition counseling, including meal planning, condition-specific dietary guidance, behavior coaching, and ongoing virtual support from a Registered Dietitian.
